Dear Flood Brook Families,
We are partnering with Mountain Valley Medical Clinic to provide flu vaccines to staff and students on Monday, October 2 at Flood Brook. If you are interested in having your child participate, please fill out the google form here and print, sign, and return the consent form. For information on the flu vaccine, you can view information from the CDC here. If you do not have access to a printer, please call the office and we will send a paper copy home. Please complete google form and consent form no later than Friday, September 29, 2023.
